Civil Acts

1) Marital Status Certificates are given at the Embassy as shown in the sample attached*.

2) Lebanese Nationals residing in Poland, Latvia, Lithuania, or Estonia can register their vital records after the submission of the following documents:

1. Registration of Marriage
Registration of Marriage Application* filled out and signed.
– Original Certificate of Marriage legalized by the Ministry of Foreign Affairs of the country where the marriage took place and translated into Arabic, English or French.
– Spouse’s original Certificate of Birth (If he/she is not a Lebanese citizen) legalized by the Ministry of Foreign Affairs of the country where the birth took place and translated into Arabic, English or French.
– Husband/ Wife’s recent Lebanese I.D. card (with a photograph) as well as a photocopy of it on a paper size A4 or a recent Individual Civil Registry Extract (with a photograph) as well as a photocopy of it on a paper size A4.
– A copy of both passports (wife and husband).
– Spouse’s name in Arabic.
– All registration procedures are free of charge.

2. Registration of Birth
Registration of Birth Application* filled out and signed.
– Original Certificate of Birth legalized by the Ministry of Foreign Affairs of the country where the birth took place and translated into Arabic, English or French.
– Father’s Lebanese recent I.D. card (with a photograph) as well as a photocopy of it on a paper size A4 or a recent Family Civil Registry Record as well as a photocopy of it on a paper size A4.
– Mother’s Lebanese I.D. card or Certificate of Birth (if she is not Lebanese).
– Parents Lebanese Certificate of Marriage.
– Child’s name in Arabic.
– All registration procedures are free of charge.

3. Registration of Divorce
Registration of Divorce Application* filled out and signed.
– Original Divorce Decree legalized by the Ministry of Foreign Affairs of the country where it has been issued and translated into Arabic, English or French.
– Special power of attorney.
– Husband/Spouse recent Lebanese I.D. card (with a photograph) as well as a photocopy of it on a paper size A4 or a recent Family Civil Registry Record as well as a photocopy of it on a paper size A4.
– Lebanese Certificate of Marriage.
– Wife’s Certificate of Birth (if she is not Lebanese).
– A copy of passports of both spouses.
– All registration procedures are free of charge.
Certificate of Divorce legalization fees: 63US$.

4. Registration of Death
Registration of Death Application* filled out and signed.
– Original Certificate of Death legalized by the Ministry of Foreign Affairs of the country where it has been issued and translated into Arabic, English or French.
– Original passport of the deceased to be cancelled and given back to his family.
– Deceased’s most recent Lebanese I.D. card (with a photograph) as well as a photocopy of it on a paper size A4, or a recent Individual Civil Registry Extract (with a photograph) as well as a photocopy of it on a paper size A4, or else a recent Family Civil Registry Record as well as a photocopy of it on a paper size A4.
– All registration procedures are free of charge.
– Certificate of Death legalization fees are as follows: 10US$.
